dc.description.abstract Workplace safety is a crucial aspect that supports productivity and the sustainability of ship operations in the maritime industry. MV. EVER OUTDO, as a cargo vessel, faces significant challenges regarding the compliance of its crew with the use of Personal Protective Equipment (PPE) during maintenance work. This research aims to identify effective strategies for fostering discipline among ship crew in PPE usage and to analyze the efforts and actions of ship officers in optimizing PPE utilization to avoid occupational accident risks. The research employed a qualitative methodology with case study, problem-solving, and descriptive qualitative approaches. Data were collected through direct observation and documentary study on MV. EVER OUTDO. Data analysis revealed that the low discipline of crew in PPE usage stems from several factors: a very low perception of risk towards high-risk tasks, the influence of permissive social norms and peer behavior, discomfort and availability of PPE, and time pressure. A fatal near-miss incident during work at height without adequate PPE served as stark evidence of the failure in risk awareness and procedural compliance. Meanwhile, officer's safety guidance and supervision face challenges such as limited briefing effectiveness, variable supervision intensity, deficiencies in coaching skills, and the burden of dual roles. Based on these findings, the research recommends an integrated strategy to enhance crew discipline, including the implementation of weekly Fatal Risk Awareness Toolbox Talks with near-miss analysis, the development of an internal "Safety Champion" program, periodic PPE needs audits, and a firm and consistent tiered warning system emphasizing the fatal consequences of PPE neglect. For optimizing officers' safety guidance and supervision, it is recommended to implement training programs for "Officers as Key Enforcers of Fatal Safety," develop integrated Safety Observation Checklists, schedule proactive safety supervision, conduct safety case simulations, and establish officer feedback forums. It is hoped that these recommendations will significantly reduce the risk of occupational accidents and improve the safety culture on MV. EVER OUTDO. en_US
