dc.description.abstract This study aims to analyze the causes of work accidents and the suboptimal implementation of cargo hold cleaning procedures on board MV. Federal Oak, as well as to formulate optimization efforts to improve crew work safety. The research uses a qualitative approach with a descriptive analytical method. Data were collected through direct observation during sea practice, interviews with crew members, and documentation studies related to work procedures and ship safety reports. Data analysis was conducted through data reduction, data presentation, and conclusion drawing supported by root cause analysis using the 5 Why method. The results of the study indicate that work accidents during cargo hold cleaning activities were caused by the suboptimal implementation of safety procedures, such as the handover of equipment while still in operation, improper modification of equipment, and weak supervision from responsible officers. In addition, the inconsistent implementation of safety meetings, inadequate use of personal protective equipment (PPE), and limited crew understanding of safety management also contributed to the occurrence of work accidents. The Internet of Things (IoT) approach in this study is used as a conceptual framework to support a more systematic work safety monitoring system through monitoring work activities, crew working hours, and compliance with safety procedures. By optimizing the implementation of cargo hold cleaning procedures, strengthening supervision, and enhancing the safety culture among crew members, it is expected that cargo hold cleaning activities can be carried out more safely and in accordance with the applicable occupational safety standards on board MV.
