Thisstudy aimsto analyze the causes of occupational accidentsin the form of a Helmsman
slipping during hose connection activities on MT Samudera Biru 168 and to formulate
effective prevention efforts. The research method used is descriptive qualitative with a risk
assessment approach, through participatory observation, in-depth interviews, and
documentation studies. The results show that the main causes of the accident were
slippery deck conditions due to residual liquid spills that were not immediately cleaned,
as well as crew fatigue due shipping schedules that reduced alertness and concentration
levels. Other contributing factors include the absence of strict spill cleaning procedures,
non-strategic placement of oil spill kits, unavailability of hazard warning signs, and weak
supervision by ship officers. Recommended prevention efforts include improving crew
situational awareness through regular safety meetings, implementing spill cleaning
procedures with a maximum time limit of 5 minutes, placing oil spill kits in strategic
locations near the manifold, providing hazard warning signs and drip trays, improving
lighting in the work area, applying anti-slip coating on the deck, as well as re-evaluating
shipping schedules to ensure crew get adequate rest time in accordance with STCW Code
standards. This research is expected to provide input for shipping companies in
improving the safety management system on board vessels. |
